<p>Cal/OSHA authorities and fair officials are now trying to determine how or why the injurious workplace accident occurred.</p>
<p>According to reports the worker is an East Bay resident in his mid-twenties.  The accident occurred at approximately 2:20 pm. when the employee was standing on the tracks of the &#8220;Wacky Worm&#8221; roller coaster.  The roller coaster slammed into the employee.  It is unknown why the employee was standing on the tracks.  The tracks are elevated about 5 feet above the ground.  EMS immediately responded to the scene and rushed the injured worker to Santa Rosa Hospital for treatment and surgery.   Reports indicate he was released from the operating room at 9:30 p.m Wednesday.</p>
<p>Harry Mason, CEO of Brass Ring Amusements Midway of Fun said, &#8221; No one can figure it out.  He was not supposed to be in that restricted area.  We&#8217;re hoping for a speedy recovery.&#8221;  He also stated that he wished to speak with the injured worker as soon as possible, in an attempt to figure out why the incident might have occurred. The victim was apparently employed as a ticket collector on another ride at the Petaluma fair.  Cal/OSHA is expected to conduct a formal probe into the on-the-job mishap.</p>

<p>The county&#8217;s Mental Health Crisis Team consoled members of Lira&#8217;s crew, witnesses and others involved with the accident.</p>
<p>&#8220;We are very sorry to announce the death of a longtime county employee who died this morning in a tragic accident,&#8221; John Presleigh, director of Public Works, and Susan Mauriello, county administrative officer, said in a prepared statement. &#8220;We are deeply saddened by this incident, and our hearts and condolences go out to family and friends.&#8221;</p>
<p>The accident is still being investigated by the county Public Works Department, Sheriff&#8217;s Office, and California Division of Occupational Safety and Health.</p>

<p>According to Grovenstein, the incident happened at the Zeta Beta Tau fraternity house at the intersection of 6th Street and Techwood Drive at approximately 12:30 a.m. According to Michael Parker, an engineering student, the men were reportedly burned as they tried to mix aluminum powder and rust powder in an experiment called &#8220;high school basic chemistry.  Parker was at a fraternity house across the street when the accident happened.</p>
<p>Parker stated that the experiment that Keen and Grzybowski were performing can produce a pyrotechnic effect similar to a huge sparkler and the resulting reaction &#8220;runs thousands of degrees Celsius, it&#8217;s molten iron.&#8221;</p>
<p>The accident is still under investigation and the Environmental Health and Safety Team is working closely with the Atlanta Fire Department to determine exactly what materials were used in the incident and how the injuries occurred.</p>

<p>Koppel lived in Queens with his girlfriend and daughter.</p>
<p>The man drinking with Koppel was Russell Wimberly according to the New York Post.  Wimberly was drinking with Koppel throughout the night and stated that Koppel was drinking whiskey and neither of them had anything to eat all day long.</p>
<p>Wimberly and Koppel ended the night at the the apartment that belonged to Wimberly&#8217;s friend and told Koppel to sleep off his drunken state.  At the apartment Koppel got sick and wasn&#8217;t breathing when Wimberly discovered him sometime after they had arrived at the apartment.  Wimberly and the apartment owner called 911.</p>
<p>The death is still being investigated and drugs and/or alcohol are suspected to be contributing factors.  There were no immediate signs of criminality.</p>

<p>The driver of the vehicle, Wilson, turned himself in later during the day to officials.  He was then charged with six counts of felony hit-and-run.</p>
<p>The bicyclists were a part of a cycling team called the Gary Fisher Subaru Team.  The names and conditions of the cyclists were not disclosed.</p>
<p>North Carolina laws state that bicycles are considered to be vehicles and the y are required to ride &#8220;predictably and courteously&#8221; to keep traffic moving and to avoid accidents.</p>
<p>The reason behind Wilson hitting the pack of cyclists is still unknown and it has not been determined yet if drugs and/or alcohol were involved.</p>

<p>It is not clear who had the green traffic light at the time of the collision according to officials reports.  The patrol car did not have its lights or sirens engaged when they crossed through the intersection.</p>
<p>Firefighters responded to the accident scene and removed both officers from the wrecked vehicle.  The officers were then transported by ambulance to Memorial Hermann Hospital for treatment.  Neither of the officers have life-threatening injuries.</p>
<p>The driver of the SUV and his 2-year-old passenger were uninjured in the accident.  A sobriety test was given to the SUV driver and he passed all tests.</p>
<p>The accident is still under investigation.</p>

<p>Brewers spokesperson, Tyler Barnes, would not release specific details of the incident or the injuries to the man due to privacy laws.  Head Athletic director for the Brewers noted the injured fan did sustain lacerations to his face but was conscious when receiving medical transport.</p>
<p>The accident is still under investigation and a current condition status on the injured fan had not been released.</p>

			<content:encoded><![CDATA[<p>The body of James P. O&#8217;Brien, 47,  was identified Wednesday by State Police.  He had apparently drowned in the Hubbard Brook in Grandville State Forest.  O&#8217;Brien was from Westfield.</p>
<p>According to State Police Lt. David Wilson, O&#8217;Brien was located in a remote area of the forest Tuesday afternoon.</p>
<p>O&#8217;Brien was a teacher at Roger L. Putnam Vocational Technical High School in Springfield.</p>
<p>Trooper Gary Fitzgerald suggested that O&#8217;Brien drowned after a kayaking accident. He was kayaking with friends on Hubbard Brook .  O&#8217;Brien was known as one of the most skilled, experienced and safety conscious white water kayakers in the area by many in the Western Massachusetts kayaking community.  O&#8217;Brien had a passion for whitewater rafting that was only matched by the love of teaching his students at Putnam.</p>
<p>O&#8217;Brien was described by Kevin McCaskill, Putnam principal, as “a consumate professional and just beautiful man.” He said O’Brien was “great, great with students, always engaging.”</p>
<p>O’Brien and teacher Daniel M. Meehan ran the school’s Putnam Outdoor Adventure Club. They took students whitewater rafting, kayaking, fishing, mountain biking, cross country skiing, camping and rock climbing.</p>

<p>At the time of the accident Storch was wearing a helmet.  Her father described her as an “amazing athlete.”</p>
<p>The ski resort released a statement saying the accident occurred on an intermediate trail.  Ski Patrol responded immediately at 4:20 when notified.  &#8221;Advance life support&#8221; was provided by the Ski Patrol and Storch was taken to the Vail Valley Medical Center.</p>
<p>She was later transported to St. Mary’s Hospital in Grand Junction, where she was listed in critical condition as of Monday evening.  Bettis told The Denver Post that Taylor was declared brain dead at 12:15 p.m. Monday.</p>
<p>Taylor&#8217;s father, Todd Storch wrote on his Twitter account Monday, “Words cannot begin 2 explain our sorrow, sadness and helplessness.  God gave us Taylor for 14 years. To know her or of her is a blessing.”</p>
<p>Taylor&#8217;s family has made the decision to donate her organs.</p>
